What you will do
Provide direct nursing care in the home setting
Assess patient condition and make clinical decisions
Follow and implement the plan of care
Monitor for changes and report to care team
Document visits and patient updates in a timely manner
Collaborate with RN Case Manager and leadership
Maintain clear communication with patients and families
What you bring
Active RN license
1+ year of clinical nursing experience
Strong assessment and critical thinking skills
Dependable transportation and valid driver's license
Ability to work independently and manage your day
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
