Be the first trusted connection for patients and families entering hospice care
Lead the admissions process with confidence, compassion, and professionalism
Perform in-depth assessments and create individualized plans of care
Educate and empower families so they feel supported, informed, and at ease
Partner with physicians and interdisciplinary teams to ensure seamless care
Provide real emotional support-not just clinical care
Coordinate services and guide patients through every step of their journey
Mentor and supervise support staff to maintain exceptional care standards

At least 1 year of nursing experience (hospice or home health a plus)
Excellent communication and interpersonal skills
Strong organizational and critical thinking abilities
Ability to work independently while thriving in a team environment
Willingness to travel and take after-hours calls
Ability to lift up to 55 pounds and perform physical job duties
Commitment to compassionate, patient-centered care
