Über

The Family Nurse Practitioner will deliver comprehensive primary care services to individuals and families across the lifespan. Responsibilities include conducting health assessments, diagnosing and managing acute and chronic conditions, prescribing medications, and providing patient education focused on health promotion and disease prevention. The FNP will collaborate with a multidisciplinary team to ensure holistic and coordinated care tailored to the diverse needs of the community served. Requirements
Perform comprehensive health assessments, physical exams, and patient histories. Diagnose and manage acute and chronic illnesses across all age groups. Develop and implement individualized treatment plans, including prescribing medications. Provide health education and counseling to patients and families. Collaborate with healthcare professionals to coordinate patient care. Maintain accurate and timely documentation of patient encounters and care plans. Stay current with evidence-based practices and participate in ongoing professional development.
EDUCATION REQUIRED:
Master's Degree in Nursing with specialization as a Family Nurse Practitioner.
EXPERIENCES AND/OR SKILLS REQUIRED:
Minimum of two (2) years experience in primary care or a related nursing field. Proficient in managing diverse patient populations across the lifespan. Strong clinical decision-making and diagnostic skills. Excellent interpersonal, communication, and organizational skills. Familiarity with electronic health records and standard medical software.
LICENSES/CERTIFICATIONS REQUIRED:
Current certification as a Family Nurse Practitioner (FNP-BC or equivalent). Licensed to practice as a Nurse Practitioner in New York State. Current CPR/BLS certification.
