What You'll Do
Partner with new and long-term residents to evaluate needs, develop personalized plans of care, and deliver effective, evidence-based therapy services. Provide education and hands-on training to residents, families, and caregivers on functional skills essential to health, safety, and wellbeing. Collaborate with an interdisciplinary care team to ensure each resident receives comprehensive, resident-centered support. Identify when referrals to other disciplines or external providers are needed to support the whole-person care approach. Conduct screenings, assessments, and evaluations-including reviewing history, completing appropriate tests and measures, and interpreting findings to create meaningful goals and treatment plans. Adapt care plans as needed to ensure progress, safety, and individualized care. Integrate wellness, health promotion, and functional activities into treatment to support long-term outcomes. Educate and assist residents with adaptive equipment, orthotics, and prosthetics as needed. Perform routine reassessments to adjust and progress treatment. Maintain accurate, timely clinical documentation and support the department's scheduling and workflow expectations. Participate in resident care conferences, family meetings, and team discussions. Provide supervision to PTAs, temporary licensees, students, and support staff in alignment with licensure and professional standards. Promote a safe, supportive, and respectful environment for all residents. Ensure the responsible and effective use of facility equipment and resources. Who We're Looking For Someone who:

Ability to work independently, handle specific deadlines and several tasks simultaneously Ability to learn and independently manage multiple medical electronic documentation systems and basic systems such as Outlook, Microsoft Word, Excel and 365 Office applications
