We are seeking a PRN Physical Therapist who is passionate about delivering exceptional, one-on-one patient care and contributing to a supportive, team-focused environment. This role offers flexibility and the opportunity to support our Schaumburg clinic and surrounding Northwest suburban locations. Perform comprehensive initial evaluations in accordance with APTA standards Develop and implement individualized plans of care based on evaluation findings Establish and track short- and long-term patient goals Deliver hands-on, one-on-one therapy to optimize patient outcomes Monitor patient progress and communicate effectively with referring providers and care teams Educate patients through customized home exercise programs Manage discharge planning and ensure continuity of care Maintain compliance with payer and documentation standards Stay current with best practices through continuing education and professional development Collaborate with healthcare providers, case managers, and insurance partners Specialty Areas
