Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies. Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity. Provide accurate and timely drug information to physicians, nurses, and other healthcare professionals. Educate patients on their medications, including proper administration, potential side effects, and drug interactions. Conduct comprehensive medication reviews and provide MTM services to patients with chronic diseases. Participate in antimicrobial stewardship programs to promote the appropriate use of antibiotics and prevent antimicrobial resistance. Monitor for and report adverse drug reactions.
Job Requirements
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree required. 1 year of pharmacist experience required. An active State Pharmacist License in good standing in the state in which the employee works (RD PHARM) required. AHA BLS required. Fire Card (must obtain within 90 days of start date) required.
