Perform patient assessments, including medical history and physical examinations. Diagnose and manage acute and chronic conditions. Order and interpret diagnostic tests. Prescribe medications within scope of practice. Develop and implement evidence-based treatment plans. Perform clinical procedures, including immunizations, wound care, and minor procedures. Provide preventive care and screenings. Educate patients on health conditions, treatment plans, and preventive care. Promote wellness, disease prevention, and healthy lifestyle changes. Maintain accurate documentation in the electronic health record. Collaborate with physicians, providers, and interdisciplinary care teams. Follow regulatory requirements, supervision agreements, and clinical standards.

Work Environment This role operates in both clinical and administrative settings with frequent interaction with providers, staff, patients, and leadership. The position requires flexibility, strong time management, and the ability to provide compassionate care in a collaborative, fast-paced, mission-driven healthcare environment. Occasional travel between clinic sites may be required. Requirements We are looking for a provider who delivers care with compassion, professionalism, and strong clinical judgment. The successful candidate will be able to build trust with patients, communicate clearly with care teams, and support a culture of accountability, quality, and respect. Required Qualifications
Graduate of an accredited Nurse Practitioner program Current Ohio RN license and Certificate of Authority Board certification through ANCC or equivalent DEA license and prescriptive authority BLS certification Strong clinical judgment, communication, and documentation skills Ability to work effectively in a team-based care environment
Preferred Qualifications
Experience in primary care or community health Experience with Epic or a similar electronic health record system
