Schedule: Monday–Friday. Option of four 10½-hour shifts or five 8½-hour shifts. Practice Setting: Hospital Outpatient Department
Position Overview: The Stroke APP will work closely with a Stroke Neurologist who provides care across the inpatient and outpatient continuum. The APP will help ensure a seamless transition from hospitalization to outpatient follow-up, providing timely access for patients following stroke, TIA, and other cerebrovascular events. The position is focused on:
Rapid-access post-discharge stroke follow-up Secondary stroke prevention Longitudinal cerebrovascular disease management Vascular risk factor optimization Patient and caregiver education Expansion of stroke services across the region Development of innovative stroke prevention and vascular health initiatives
The APP will play a vital role in reducing readmissions, improving patient outcomes, enhancing continuity of care, and expanding access to specialty stroke services. Core Clinical Responsibilities:
Outpatient Stroke Clinic
Evaluate and manage patients following hospitalization for: Ischemic stroke, Transient ischemic attack (TIA), Intracerebral hemorrhage, Cerebrovascular disease, Cryptogenic stroke, Small vessel disease, Carotid and intracranial vascular disease Obtain comprehensive medical histories and perform neurological examinations Monitor neurological recovery and functional outcomes Develop and implement individualized care plans Formulate differential diagnoses and treatment recommendations Review and interpret neuroimaging, laboratory studies, and diagnostic testing Coordinate specialty referrals and diagnostic evaluations Collaborate with physicians and multidisciplinary teams to optimize patient outcomes
Secondary Stroke Prevention
Manage modifiable vascular risk factors including: Hypertension, Diabetes mellitus, Hyperlipidemia, Obesity, Metabolic syndrome, Tobacco use, Obstructive sleep apnea Optimize evidence-based stroke prevention therapies Monitor medication effectiveness and adherence Provide lifestyle modification counseling and risk-reduction education
Care Coordination and Transitions
Facilitate smooth transitions from inpatient hospitalization to outpatient care Ensure timely post-discharge follow-up Coordinate rehabilitation, home health, therapy services, and specialty consultations Collaborate with primary care providers and community resources Support patients and families throughout the recovery process
Program Development and Growth
The Stroke APP will have the opportunity to participate in the growth and development of a comprehensive stroke program designed to improve access and outcomes across the region. Future areas of program development may include: Stroke prevention initiatives, Community outreach and education, Vascular health programs, Lifestyle medicine integration, Metabolic health and obesity management collaborations, Population health strategies targeting recurrent stroke reduction, Expansion of services to rural and underserved populations The APP will be encouraged to participate in quality improvement projects, clinical innovation, and multidisciplinary program development efforts.
Patient and Family Education
The APP will provide comprehensive education regarding: Stroke warning signs and emergency response, Medication management and adherence, Secondary stroke prevention, Lifestyle modification, Nutrition and exercise recommendations, Caregiver support resources, Recovery expectations and rehabilitation planning
Qualifications:
Current Illinois license as a Nurse Practitioner or Physician Assistant National certification as an APRN or PA Master's degree or higher in Nursing or Physician Assistant Studies BLS certification required Neurology, stroke, hospital medicine, emergency medicine, or critical care experience preferred Strong clinical assessment and communication skills Ability to work independently while collaborating effectively within a multidisciplinary neuroscience team

Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.
