Conduct in-person patient assessments in their homes Develop individualized care plans for caregivers to follow Maintain accurate documentation within our online charting system Communicate regularly with the Eau Claire office for updates and support Manage aspects of patient care, including case coordination Travel up to one hour to complete patient assessments
Requirements
Active Registered Nurse (RN) license in the state of Wisconsin Previous healthcare experience preferred, but not required Strong communication and interpersonal skills Ability to work independently and manage time effectively
