Establishes rapport and develops therapeutic relationships with patients of diverse backgrounds and varying levels of motivation for change. Demonstrates knowledge of the full range of community resources to properly guide and direct patients. Renders accurate diagnoses utilizing the DSM-5-TR and applies evidence-based treatment guidelines. Documents and presents completed assessments for clinical disposition in a clear and organized manner. Provides consultation to and coordinates patient care with multidisciplinary staff in clinic and in the BHUCS. Maintains active DEA registration and fulfills all federal and state requirements for prescribing controlled substances, including medications for opioid use disorder. Provides urgent coverage and crisis intervention for referred patients. Conducts suicide risk assessments and safety planning in accordance with organizational protocols Screens for domestic violence, substance use and risk behaviors with every patient, applying protocols for intervention with positive screenings. Performs and documents outreach activities for patients who do not follow through, especially for those with known risk factors. Serves as an expert consultant and/or supervisor to PACE providers (Program of All-Inclusive Care of the Elderly) for their psychiatrically complex patients, consistent with experience, if needed. Demonstrates capacity to diagnose and effectively treat a range of behavioral health conditions, including anxiety, depression, adjustment disorders and moderate-severe personality disorders with evidence-based psychopharmacology. Demonstrates cultural humility and trauma-informed care principles in all patient interactions. Demonstrates excellent, advanced triage skills, assigning the appropriate level of priority at all times. Provides education and supervision to the medical and behavioral health providers regarding psychopharmacological treatment and patient management. Performs advanced, high quality, thorough psychosocial evaluation that contains data in support of the diagnosis, findings, recommendations and plans. Meets the departmental documentation standards in a timely fashion and completes documentation within 24–48 hours of service delivery to support billing compliance and continuity of care. Serves as an expert consultant and provides ongoing psychopharmacologic care to SCO (Senior Care Options) providers for their psychiatrically complex patients, consistent with experience. Demonstrates excellent clinical judgement in assessing changes in risk, enlisting the Administrative and/or Medical Directors and initiating Special Clinical Reviews as indicated. Attains working knowledge of all departmental policies and procedures and interprets them to new staff and assigned supervisees. Applies short-term, solution-based and evidence-based pharmacotherapies to patients in the BHUCS. Manages a full caseload consisting of patients seen for psychopharmacological consultation to medical providers, assessment, and medication management. Maintains professional licensure, accumulating the required number and type of CMEs and submits the associated documentation to the Medical Director. May accept responsibility for and effectively manage a departmental project, procedure or new function. Presents clinical cases for review at daily and weekly clinical meetings according to departmental standard. Maintains productivity expectations in alignment with organizational benchmarks while ensuring quality and patient-centered care. Consistently achieves direct care target of 80%, regularly analyzing with Medical Director direct care data, no show rate, and access and makes adjustments accordingly. Masters each computer-based application required for effective performance of the functions associated with the job description. Demonstrates proficiency in the electronic health record (EHR) system and participates in quality improvement initiatives utilizing EHR data. Provides consultation, training and supervision to departmental staff, medical providers, BH interns and PMHNP residents and students as needed. Successfully manages all aspects of the particular assigned program or departmental function. Performs all the required steps to ensure the generation of clean claims – completing documentation, submitting encounters on time, and completing re-authorizations. Orders and interprets appropriate laboratory tests, radiologic tests and EKGs with the support of the SEM. Monitors metabolic parameters and other safety labs in accordance with psychopharmacology best practices.
