Evaluate residents to determine speech therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies. Develop, implement, and update individualized treatment plans to improve communication, cognition, swallowing function, and overall independence. Assist the nursing department with training of Restorative Aides. Supervise Speech-Language Pathologist Clinical Fellows (CFY) and students in resident care and related activities in accordance with state practice regulations. Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding resident progress, goals, and discharge planning. Participate in Resident Care Conferences, Utilization Review meetings, and Rehabilitation meetings as needed. Provide in-service education and training programs for facility staff. Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements. Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols. Educate residents, families, and caregivers regarding treatment goals, home programs, maintenance strategies, safety techniques, and aspiration prevention. Recommend and facilitate ordering of appropriate durable medical equipment and adaptive devices to support resident independence and safety. Contribute to quality assurance initiatives, performance improvement programs, and rehabilitation program development. Establish and maintain positive professional relationships with residents, families, staff, and healthcare providers.
Supervisory Responsibilities
Supervise Speech-Language Pathology Clinical Fellows (CFY) and student clinicians, when applicable.
Qualifications
Master's Degree in Speech-Language Pathology from an accredited program. Current state Speech-Language Pathologist license in good standing. Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates may be considered. Experience in skilled nursing, post-acute care, or rehabilitation settings preferred. Strong communication, documentation, and interdisciplinary collaboration skills.
