Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting Implement therapeutic interventions to improve patient mobility, strength, and functional independence Monitor and document patient progress, adjusting treatment plans as clinically appropriate Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning Educate patients and families on home exercise programs and safe functional techniques Required Qualifications
Active Physical Therapist (PT) license in the state of Wyoming or eligibility for licensure Graduate of an accredited Physical Therapy program Current CPR/BLS certification Experience working in a skilled nursing facility or similar post-acute care environment Preferred Experience / Skills
Prior experience in a SNF or long-term care setting Familiarity with MDS documentation and Medicare guidelines in a post-acute environment Why Join Us
