Evaluate and treat patients in a skilled nursing facility setting Develop and implement individualized physical therapy treatment plans Monitor and document patient progress toward established therapy goals Collaborate with interdisciplinary care teams to ensure comprehensive patient care Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards Required Qualifications
Active Physical Therapist (PT) license in the state of Minnesota or eligibility to obtain Graduate of an accredited Physical Therapy program Experience working in a skilled nursing facility or long-term care setting Ability to fulfill a 13-week contract commitment Preferred Experience / Skills
Prior SNF or post-acute care experience Familiarity with Medicare documentation and compliance requirements Why Join Us
