Estimated Gross Weekly Pay: $2960 $500 CONTRACT COMPLETION BONUSOpportunity: Physical Therapist - Physical Therapy Job Details: • Work Monday through Friday, 8:00 AM to 5:00 PM schedule • 100% remote position within designated coverage area after initial 2-day orientation in Oak Brook, IL • See 6 patient visits per day within assigned coverage area • Provide care for adult/geriatric general medical/surgical patients • Current patient census approximately 200 patients • All meetings held virtually with no physical office to report to • Must provide own reliable transportation to/from all patient visits • Valid driver's license and auto insurance required • 2 years physical therapy home health experience with adult/geriatric patients required • Active Illinois PT license and BLS (AHA) certification required • Master's degree in Physical Therapy required • Must assist with activities of daily living (ADL's) and adaptive equipment • Ability to collaborate effectively with other specialties including RN, OT, and Speech therapy • Supplies and equipment delivered to traveler's home • EMR system: EPIC • Dress code: Any solid color scrubs or business casual attire acceptable, no jeans or open-toe shoes • No weekend rotation, call requirements, or holiday expectations • Coverage area includes zip codes: 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas Facility: This home health services organization provides comprehensive healthcare services directly to patients in their homes throughout the Chicago metropolitan area.
