Estimated Gross Weekly Pay: $3200 $500 CONTRACT COMPLETION BONUSOpportunity: Physical Therapist - Physical Therapy Job Details: • Work Monday through Friday from 8:00 AM to 5:30 PM • 13-week contract assignment starting ASAP • 37 guaranteed hours per week with flexibility up to 40 hours • Must reside at least 50 miles away from facility • Occasional travel to other branches as needed • Generally no weekends, with optional Saturday rotation every 6-8 weeks (compensated with weekday off) • No holiday requirements • No on-call or call-back responsibilities • Mileage reimbursed at government approved rate • Required certifications: BLS (AHA or ARC), Wisconsin PT License, NPI number • Master's Degree in Physical Therapy required • Minimum 2 years of home health experience highly preferred • Experience with adult/geriatric population and variety of orthopedic, neurological, and functional mobility diagnoses • Use laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication • Evaluate, treat, and plan patient care according to Physical Therapy standards • Meet productivity expectations of average 4.5 visits per day • Document to meet regulatory and reimbursement requirements • Contract eligible for 36 hours of unbillable low census hours Facility: This home health organization provides comprehensive physical therapy services to adult and geriatric patients in their homes.
