Schedule Day Shift: 5x8 Hour Schedule (08:00 - 16:30) Job Description Seeking an experienced Home Health Physical Therapist to provide exceptional patient care in the Medford, OR area. The ideal candidate will work with a manageable caseload, providing personalized therapy to patients in their homes. ResponsibilitiesDeliver individualized therapy interventions based on evaluations and patient needs. Administer treatment plans to optimize patients' functional mobility and overall health. Conduct patient assessments and document care in the electronic health record (EPIC/Oasis). Provide education to patients and families regarding home exercise programs and rehabilitation techniques. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Required Experience / Certifications / LicensureMinimum of 1 year of skilled home health experience. Current Physical Therapist license for the state of Oregon. Basic Life Support Certification (BCLS) required. Experience with Oasis is required. Why ARMStaffing?
