Follow established care plans developed by the Registered Nurse (RN) Perform skilled nursing visits in patients' homes Assess patients and monitor their condition Administer prescribed medications and treatments Provide wound care and disease management education Reinforce patient and caregiver teaching Accurately document patient care and clinical observations Communicate changes in patient condition to the supervising RN and physician Collaborate with an interdisciplinary team to ensure optimal patient outcomes Qualifications
