What You'll Do
Coordinate and manage the admissions process from referral through admission Conduct clinical reviews and assessments to support admission decisions Collaborate with hospitals, physicians, case managers, residents, and families Ensure all admission documentation is completed accurately and in compliance with regulatory requirements Facilitate smooth transitions into the facility while providing exceptional customer service Serve as a resource for residents, families, referral partners, and members of the care team Help drive census growth through strong relationship management and responsiveness
What We're Looking For
Current, active Registered Nurse (RN) license in the State of Hawaii Strong clinical assessment and critical thinking skills Ability to make sound independent decisions Knowledge of nursing practices, medical procedures, and long-term care regulations Excellent communication, customer service, and interpersonal skills Ability to work effectively with an interdisciplinary team Strong organizational, analytical, and problem-solving abilities Positive attitude and genuine passion for serving older adults Recent geriatric nursing experience required Long-term care experience preferred
