Provide supervision, assessment, planning and direct patient care to hospice patients.
Perform accurate and complete admissions assessments and re-assessments.
Assess patients for safety risks, abuse and neglect.
Assess, document and manage patient's pain.
Prioritize and document patient problems.
Administer medications and treatments safely and accurately with a focus on patient comfort.
Coordinate care for managing a caseload of patients.
Document changes in patient condition and effectiveness and timeliness of care/interventions.
Evaluate, update and revise plans of care.
Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of Nursing
Current licensure as a Registered Nurse (RN) in the State of Ohio
Basic Life Support (BLS) Certification from the American Heart Association (AHA)
One year of current clinical experience as an RN
One year of experience in an acute medical-surgical or critical care setting
Valid Driver's License (VDL) and proof of automobile insurance with $100,000/$300,000 coverage required
Preferred qualifications for the ideal future caregiver include:
Bachelor's of Science in Nursing (BSN)
Hospice and Palliative Care Nurse Certification
Physical Requirements:
Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination.
Requires standing and walking for extensive periods of time.
Heavy work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
Personal Protective Equipment:
Follows standard precautions using personal protective equipment as required.
Pay Range Minimum hourly: $35.50 Maximum hourly: $55.60 The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.). The pay range does not include the value of Cleveland Clinic's benefits package (e.g., healthcare, dental and vision benefits, retirement savings account contributions, etc.).
