We are seeking an RN Care Coordinator to coordinate, implement, and evaluate participant-centered care within the PACE interdisciplinary team (IDT). This role ensures participants receive comprehensive, high-quality, and cost-effective care across the continuum of services. Responsibilities include conducting assessments, developing individualized care plans, overseeing clinical interventions, and collaborating with physicians, social workers, therapists, home care staff, and other care team members. Responsibilities Conduct nursing assessments, develop and update individualized care plans, and monitor participant needs including chronic disease management, medications, safety, ADLs/IADLs, mental health, and caregiver support. Coordinate care across clinic, home care, specialty, hospital, and long-term care settings, ensuring seamless communication and transitions for participants, families, caregivers, and providers. Provide direct nursing care including skilled visits, triage, wound care, and medication administration, while monitoring treatment responses and collaborating with the IDT on care plan adjustments. Serve as the primary nursing contact for assigned participants and collaborate with physicians, social workers, therapists, dietitians, and home care staff to deliver comprehensive, coordinated care.
Qualifications Education: Degree/diploma in nursing
Experience Two years clinical nursing experience preferred in hospital or nursing facility setting
License(s)/Certification(s): Maintains current registered nursing license in the State of Iowa as required. Maintains current CPR status Maintains current Mandatory Reporter Valid driver’s license when driving any vehicle for work-related reasons. Valid Auto insurance.
