Pharmacist Job Specifics and Requirements: Area: Must be able to work near Sulphur, LA Licensure Requirements: Must have open, active, and in good standing LA State Pharmacist License Education: Pharm. D. Experience Needed: 1+ years of previous pharmacist experience. Schedule: Full-Time Pay: $114,400-$145,600 DOE Background/ Drug Requirements: Ability to successfully complete background check, health and drug screen Pharmacist Job Responsibilities:
Review and verify medication orders for long-term care facility residents to ensure accuracy and appropriateness Conduct comprehensive drug regimen reviews and identify potential medication-related issues Collaborate with physicians, nurses, and healthcare teams to optimize patient medication therapy Monitor for drug interactions, contraindications, and adverse effects Ensure compliance with federal and state regulations governing long-term care pharmacy operations Provide consultant pharmacist services and recommendations to facility staff Participate in interdisciplinary care planning meetings and provide clinical expertise Maintain accurate documentation and records in accordance with regulatory requirements Educate healthcare staff on proper medication administration and storage procedures Respond to medication-related inquiries and provide drug information support Oversee medication storage, handling, and disposal protocols within the facility Conduct quality assurance activities and participate in continuous improvement initiatives
