Conduct evaluations for patients in both inpatient and outpatient environments, crafting personalized treatment plans tailored to individual needs Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Collaborate effectively with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation specialists to ensure optimal outcomes Educate patients and their families about therapeutic objectives, discharge planning, and at-home exercise routines Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility and regulatory requirements Engage in care team discussions and interdisciplinary conferences to promote seamless care Education
Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications
State licensure as a Physical Therapist CPR certification preferred Specialized credentials (e.g., neurological, geriatric) are advantageous but not mandatory Skills
