Hospital Pharmacist (Contract) - Day/Evening Rotating ShiftsLocation: Glenwood Springs, Colorado Facility: Valley View Hospital Job Type: Contract Assignment Start Date: ASAP Assignment Length: Approximately 20 Weeks Schedule: 40 Hours Per Week | Rotating Day/Evening Shifts | Rotating Weekends Position Overview Valley View Hospital is seeking an experienced Hospital Pharmacist to join its pharmacy team on a contract basis. The ideal candidate will possess strong clinical pharmacy experience, proficiency with Epic, and an active Colorado Pharmacist License. This position offers the opportunity to work in a collaborative healthcare environment focused on delivering safe, effective, and patient-centered pharmaceutical care. Schedule Details
Rotating day and evening shifts 40 hours per week guaranteed Rotating weekends (7:00 AM - 12:00 PM) Occasional overnight shifts may be required based on departmental needs Holiday coverage required
Key Responsibilities
Interpret, verify, and process medication orders accurately Maintain computerized patient medication profiles Prepare and dispense medications, including IV admixtures, chemotherapy agents, and parenteral nutrition products Monitor medication therapies for safety, efficacy, contraindications, and drug interactions Perform pharmacokinetic dosing in accordance with hospital protocols Review culture and sensitivity data and provide antimicrobial stewardship recommendations Evaluate patient charts and clinical data to support optimal medication management Detect, document, and report adverse drug reactions Provide clinical consultations and medication recommendations to physicians and healthcare staff Educate patients and families regarding medication therapies Document clinical interventions and pharmacy activities accurately Participate in quality improvement initiatives and departmental audits Assist with staff education, orientation, and training programs Serve as a member of the Code Response Team Maintain compliance with all federal, state, and local regulations
Required Qualifications
Active Colorado Pharmacist License in good standing (required at time of submission) Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree or equivalent Minimum of 3 years of pharmacist experience Epic experience required Current BLS Certification (American Heart Association) Current ACLS Certification (American Heart Association)

Additional Requirements
