Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways. Utilizes assessment data to identify patient care needs, to include patient education and discharge planning. Recognizes abnormal assessment data and incorporates findings into the plan of care: validates and revises expected outcomes based on assessment. Organizes and prioritizes patient care activities to include procedures, therapies, tests and patient education to achieve the desired outcomes. Engages in basic caring practices to include patient education focusing on psychosocial and physiological needs and serves as a resource to the patient for immediate healthcare needs. Provides compassionate and empathetic hands on care and maintains patient privacy and confidentiality. The incumbent is a professional caregiver who assumes responsibility and accountability for a patient assignment through therapeutic use of self, the nursing process, and other health care team members. The incumbent, as a member of the health care team appropriately assists team members in providing holistic care to patients and their support systems with an awareness of cost containment and time management. #RN@DH Job RequirementsRequired
