Key Responsibilities
Deliver ongoing, comprehensive care for patients of all ages, from children to the elderly. Address chronic diseases, preventive health, acute conditions, and mental health issues using a patient-centered approach. Conduct thorough physical assessments, analyze diagnostic data, and formulate individualized care plans. Collaborate closely with care coordinators, mental health experts, and specialists to provide comprehensive patient care. Engage in quality improvement initiatives that enhance patient outcomes and operational performance. Leverage electronic medical records (EMR) to ensure thorough and current patient documentation. Provide care options both in-person and through virtual consultations to meet diverse patient needs. Education Requirements
MD or DO from an accredited medical institution is required. Completion of a recognized Family Medicine residency program. Certifications
Board Certification in Family Medicine. Active California medical license (or eligibility to obtain one). BLS and ACLS certifications are preferred. Qualifications and Skills
Demonstrated diagnostic and clinical expertise, with a focus on preventive healthcare. Experience with value-based care models and a passion for continuous learning. Outstanding communication, teamwork, and interpersonal abilities. Technologically adept, proficient in EMR systems and telemedicine tools.
