What You'll Do As the LPN Care Coordinator, you'll play an important part in ensuring residents receive exceptional care each day while working within a supportive interdisciplinary team. Your responsibilities include:
Assist each client to reach and maintain his or her highest level of performance and independence by performing assistance with activities of daily living and implementing scheduled services as needed Participates with registered nurses in the development and review of care plans; participates in care conferences Assist with the admission process as directed Perform safe administration of medications and implementation of treatments and other therapies as directed Completing documentation with accuracy and attention to detail Supporting Resident Assistants through mentorship, coaching, and positive leadership Managing medication orders, refills, and overall medication processes Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License Demonstrate effective written and verbal communication with residents, family members, staff, and other health care providers. Maintain service excellence and a high level of confidentiality. Actively participate in agency-wide activities and its continuous quality improvement Understand the importance of seeking assistance as needed and demonstrate dependability and capability of following instructions. Adhere to policies, and procedures and participate in the organization's quality improvement program.
