Evaluate and treat patients in a skilled nursing facility setting to restore and improve physical function Develop and implement individualized physical therapy treatment plans based on patient needs and goals Monitor and document patient progress, adjusting treatment plans as clinically appropriate Collaborate with interdisciplinary care teams to ensure coordinated patient care Educate patients and family members on therapeutic exercises and home care strategies
Required Qualifications
Active Physical Therapist (PT) license in the state of Arizona Graduate of an accredited Physical Therapy program Experience working in a skilled nursing facility (SNF) setting Current CPR/BLS certification
Preferred Experience / Skills
Prior travel or contract therapy experience Familiarity with SNF documentation and Medicare compliance requirements
