Provide direct nursing care in the patient's home
Administer medications and treatments as prescribed
Monitor and document patient condition and vital signs
Perform skilled nursing procedures (e.g., trach care, ventilator management, G-tube feeding, wound care)
Collaborate with physicians, families, and care teams
Educate patients and families on care plans and disease management
Maintain accurate and timely documentation
Qualifications
Active RN or LPN license in Massachusetts
Current CPR/BLS certification (required)
Minimum 1 year nursing experience (home care, pediatric, or critical care preferred)
Experience with trach/vent, G-tube, or complex care (preferred but willing to train)
Strong clinical and communication skills
Reliable transportation and ability to travel to patient homes
