Affirmed Home Care, Connecticut's premier concierge home care agency, is seeking compassionate and experienced per diem pediatric LPNs to provide dedicated one-on-one care for a sweet 2-year-old boy in Greenwich, CT. This child is recovering and thriving at home with the support of skilled nursing care and a loving family. Conveniently located near the New York–Connecticut border, this opportunity is ideal for nurses residing in Westchester County, NY, and nearby communities. Case Information Location: Greenwich, CT Patient: 2-year-old pediatric client Clinical Needs: Tracheostomy care and temporary G-tube care Home Environment: Beautiful family home, smoke-free, with no pets Family: Warm, supportive, and welcoming Available Shifts
12-hour day shifts 12-hour overnight shifts flexible per diem scheduling

Qualifications
active Connecticut LPN license new York applicants must maintain active LPN licenses in both New York and Connecticut minimum of 1 year of pediatric experience in home care or acute care preferred minimum of 1 year of experience providing pediatric trach and G-tube care current BLS certification authorized to work in the United States physical exam completed within the past year PPD/QuantiFERON completed within the past year, or chest X-ray completed within the past five years
