Key Responsibilities
Comprehensive Assessments: Evaluate psychosocial status of patients and families related to terminal illness and living environmentCare Planning: Develop and implement interventions based on thorough evaluationsCounseling & Support: Guide patients and caregivers through stress, coping challenges, and end of life issuesDocumentation: Maintain accurate, timely clinical records for all referred patientsResource Navigation: Provide referrals and serve as a liaison with community agenciesTeam Collaboration: Participate in interdisciplinary care planning and regular team meetingsAdvocacy: Help physicians and team members understand social and emotional factors impacting careQuality Improvement: Actively contribute to performance improvement initiativesFlexibility: Perform additional duties as delegated by the Clinical Supervisor
Requirements
Bachelor's degree in Social Work from a CSWE-accredited programMinimum 1 year in healthcare; hospice experience strongly preferredDeep understanding of hospice philosophy and end-of-life careCurrent CPR certification requiredValid driver's license, auto insurance, and reliable vehicle for community visits
