Assess & Plan Coach & Supervise Monitor & Document Track vitals, condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
Collaborate & Advocate Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
Quality & Compliance Maintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.
What You'll BringActive Licensed Practical Nursing license in CO
2+ years' geriatric/home-care experience Solid clinical assessment skills and a passion for problem-solving CPR/First Aid certification and a valid driver's license Warm, professional communication that builds trust with families and field staff
