RN Case Manager - 13-Week Contract Position Location: Margate, Florida Assignment Duration: This is a 13-week contract position. Schedule: 0800-1630, Monday through Friday, with every other weekend required. Job Description: We are seeking a dedicated RN Case Manager to join our team. The ideal candidate will possess experience in an acute care setting and will be responsible for coordinating patient care, managing treatment plans, and collaborating with interdisciplinary teams to ensure high-quality patient outcomes. Responsibilities
Coordinate and manage patient care for a diverse population Develop and implement individualized care plans Communicate effectively with patients, families, and healthcare teams Monitor patient progress and adjust care plans as necessary Educate patients and families on disease management and resources
Required Experience / Certifications / Licensure
Active RN license in the state of Florida Experience in an acute care setting Basic Life Support (BLS) certification Ability to multitask, work independently, and maintain a positive attitude Strong communication and time management skills
