Über
Conduct comprehensive intake assessments and perform medication administration. Manage symptoms effectively through pain management techniques and interventions. Provide goal setting and education to patients and their families. Conduct symptom assessments and manage various patient diagnoses. Actively participate in a multidisciplinary team to ensure high-quality patient care.
Required Experience / Certifications / Licensure:
Minimum of 2 years of experience in Home Health, Hospice, or Skilled Nursing settings. Current BLS certification. Active RN license in the applicable state, or Compact license. Experience with medication administration and symptom management. Familiarity with EPIC charting system preferred.
