The RN Case Manager cares for patients with terminal illnesses or other diagnoses, indicating they have fewer than six months to live. The RN Case Manager will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life. The RN Case Manager is typically part of a care team. This team collaborates to ensure individuals and their families have the physical, emotional, and spiritual support they require throughout the care process. Benefits:
