Qualifications
Licensed Practical Nurse in the state of Pennsylvania
We are also open to graduate nurses awaiting licensure (pending available supervision)
Responsibilities
Coordinate, assess, plan, deliver, and evaluate nursing care for assigned residents.
Perform accurate, thorough, and timely nursing assessments in accordance with department standards.
Deliver both direct and indirect care that promotes each resident's highest level of functioning and well-being.
Evaluate residents' responses to treatment and collaborate with physicians and the interdisciplinary team to adjust care plans as needed.
Coordinate and document the discharge planning process, ensuring residents and families understand the ongoing care plan.
