Assists with the supervision of daily operations of practice nurses and patient support care navigators across multiple offices. Assist with the designs, implementation, and expansion of comprehensive support services using quality standards, training, monitoring, and feedback that is consistent with Brown University Health's strategic vision. Monitors and evaluates clinical triage and assessment competencies, recommends opportunities for improvement or expansion of services based on findings. Develops and analyzes Brown Health Medical Group Primary Care's and BHMG's outcomes data and develop plans to improve performance as it relates to contracts and other quality initiatives. Lead training and onboarding for new staff and provide ongoing education on system updates and compliance standards. Participate in cross-functional meetings to align workflows with clinical and operational goals. Collaborates with Brown Health Medical Group Primary Care and BHMG physicians and care teams on the management of patients by providing care coordination, assessment and triage, patient education and clinician support. Works collaboratively with the Clinical Quality Team and Data Analytics Team to develop and implement programs to improve healthcare workforce well-being and quality metrics. Attends required meetings. Seeks guidance and resources, when necessary, in care and treatment of patients. Comply with federal and local confidentiality laws, including HIPAA, ensuring patient privacy. Adheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients' demographic, clinical and financial information. Performs other job-related duties as assigned.
Education: Associate or bachelor's degree in nursing with current Rhode Island RN license in good standing. Experience: 3-5 years of appropriate nursing experience. Prior supervisory or team lead experience is strongly preferred. Proficiency in EMR systems (Epic), document management platforms (OnBase), and Microsoft Office Suite. Capacity to collaborate with colleagues and teammates as necessary to ensure quality work. Ability to work both on a team and independently, with the capability to self-direct. Ability to use technology with proficiency in electronic medical record (EMR) platforms and Microsoft Office applications such as: Outlook, Excel, and Word. Ability to function well in a high-paced and at times stressful environment. Capacity to multi-task, think critically and problem solve. Clear assessment, creativity, judgment, and decision-making skills. Ability to take initiative, organize, prioritize, and follow through with work assignments assuring fulfillment of plans and goals. Excellent communication skills – both verbal and written. Ability to maintain quality, safety, and/or infection control standards. Ability to perform standard clinical procedures according to established protocols. Ability to maintain professionalism and provide superior customer service to all. Supervisory responsibility: Provides oversight and direction to the Clinical Practice Team, Practice Nurses, and Patient Support Care Navigators, ensuring consistent delivery of high-quality patient care and service operations. Physical requirements: Prolonged periods of sitting at a desk or standing while using a computer. Work and move among all departments of Brown Health Medical Group. Excellent hand/eye coordination. Lift up to 15 pounds. Manual dexterity to operate a personal computer or laptop as well as standard office machines and equipment as it pertains to one's position. State-wide travel and operation of motor vehicle may be required for offsite meetings and trainings. Must have a valid driver's license and clear driving record. Use of personal vehicle and proof of auto insurance is necessary.
