Key Responsibilities
Provide skilled, in-home nursing care to assigned patients Develop and follow individualized care plans Accurately document patient care, observations, and updates Collaborate with interdisciplinary care teams Maintain compliance with DOL, CMS, and state regulatory requirements Qualifications
Current LPN license in the state of [WI] Current CPR certification Valid driver's license and reliable, insured vehicle Home health experience preferred but not required Must pass pre-employment screenings Ready to Make a Meaningful Impact?
