What You'll Do Deliver Compassionate Hospice Care
Provide direct patient care in accordance with the state Nurse Practice Act, including end-of-life care Implement and support individualized plans of care under RN supervision Deliver effective pain and symptom management, evaluating patient response to treatment Educate patients and caregivers on disease progression, comfort measures, and care needs Maintain a safe, clean, and supportive care environment
Collaborate & Communicate
Document care accurately and timely in the EMR Partner with physicians and interdisciplinary team members (RN, HHA, Social Work, Chaplain, etc.) Participate in care coordination and team discussions to ensure holistic patient support Build trusted, compassionate relationships with patients and families
Commit to Excellence
Uphold the highest standards of clinical practice and professionalism Participate in quality improvement and team initiatives Engage in ongoing professional development
What You Bring
Active LPN/LVN license in the state of practice Current CPR certification 1–3+ years of nursing experience preferred (hospice, home health, or med-surg a plus) Strong communication skills and emotional intelligence Ability to provide compassionate, patient-centered end-of-life care Valid driver's license, reliable transportation, and auto insurance
