Planning, reviewing and implementing treatment programs based upon initial and interim evaluations. Communicating and collaborating with the patients, family members, caregivers, interdisciplinary professionals and other internal/external stakeholders both orally and in writing. Adapts assessment tools and treatments to the complexity of the diagnosis or disabilities and demonstrates the clinical reasoning necessary to identify the need for further in-depth specific assessment of function and utilization of non-standard methods and techniques. Documents plan of care and care given in patient's medical record. Monitors patient's response to interventions, modifies treatment plan, and reevaluates as indicated to meet short-term and long-term goals. Provides appropriate devices for activities of daily living and upper extremity support and can fabricate, adjust, adapt, and modify orthoses, splints, and adaptive equipment or other durable medical equipment for activities of daily living in order to meet the needs of patients. Provides instruction and training to patient and/or caregiver on all equipment or assistive devices issued. Develops home programs for patients as needed prior to discharge and instructs patients, family or caregivers in implementation of home program. Provides consultation, as needed, to peers and other staff responsible for the care of the assigned patient. Provides clinical oversite and collaboration with occupational therapy assistants (OTA). Provides patient care which is appropriate to the cognitive, emotional, and chronological maturation needs of young adults (18-45 years old), middle adults (45-65 years old), and older adults (65-older) patients. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Assures section compliance with all applicable standards such as CARF and the Joint Commission and National Patient Safety Goals. Documents initial findings, treatment goals, equipment recommendations, patient progress and discharge summaries timely and in accordance with established policies. Responsible for patient education in his/her program area. Assesses patient/family knowledge-level, implements plan of instructions, and records the patient's family's demonstrated level of understanding. Maintains clinic and equipment in a safe and orderly manner in accordance with safety and infection control policies, Joint Commission, CARF regulations, and other relevant regulatory policies. Attends and actively participates in conferences, rounds, huddles, discharge planning, team meetings, etc., as part of an interdisciplinary team. Participates in discharge planning.
Requirements
Shall have a full and unrestricted license in a State, Territory, or Commonwealth of the United States of the District of Columbia. Current AHA Basic Life Support certification. Minimum two years current experience. Graduate from a program accredited by ACOTE – Accreditation Council for Occupational Therapy Education. Current National Board for Certification in Occupational Therapy (NBCOT) certification as an OT.
