Pharmacist Positions Available We are looking for three (3) pharmacists to join our various pharmacy teams at Park DuValle Community Health Center (Newburg - PIC needed, Taylorsville (Spencer County), and Wilson (Louisville). Our pharmacists provide safe, effective, and patient-centered medication services to a diverse and underserved population within a Federally Qualified Health Center. Working collaboratively with an interdisciplinary care team, the Pharmacist supports medication management, patient education, chronic disease care, and compliance with federal and state regulations, while advancing the health center's mission of access and quality. Primary responsibilities:
Review, verify, and dispense prescription medications accurately and efficiently. Assess prescriptions for appropriateness, interactions, allergies, and contraindications. Counsel patients on medication use, dosage, side effects, and proper storage. Supervise pharmacy technicians and support staff as required. Provide clinical guidance and drug information to healthcare providers. Monitor patient outcomes and recommend therapy modifications when appropriate. Maintain accurate and up-to-date patient medication records. Manage and monitor inventory, including ordering, storage, drug returns, and disposal of medications. Administer immunizations and conduct health screenings where authorized. Participate in quality improvement initiatives related to medication management. Stay current with developments in the field of pharmacy practice and adhere to all relevant laws and regulations. Performs other duties as assigned.
Requirements Minimum Requirements:
Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy. Active and unrestricted pharmacist license in Kentucky. 1-3 years of experience. Strong knowledge of pharmacology, drug therapy, and pharmaceutical calculations. Excellent communication and interpersonal skills. High attention to detail and strong problem-solving abilities. Ability to work collaboratively in a multidisciplinary healthcare team. Proficiency in using pharmacy software and electronic health records.
