Pharmacy Medication History Interns, under the direct supervision of a licensed pharmacist and in accordance with all state and federal laws to obtain and document medication histories for patients admitted to the hospital. Medication history will be obtained from the patient directly or the patient's family through patient interviews, contacting retail pharmacies or primary clinics and medication history software queries. Interns enter medication history information into the electronic medical record and communicate completion of medication history to pharmacists. Completes other projects assigned but does not perform duties that can legally be performed only by a registered pharmacist.
Additional accountabilities will include:
General knowledge of the principles and practices of professional pharmacy
Knowledge of generic and brand name terminology * Knowledge of pharmacological and legal classification of drugs
Knowledge of medical terminology and medical abbreviations
Working knowledge about common outpatient medication doses and forms.
Ability to proficiently read, write and speak English.
Ability to utilize interpersonal and communication skills required to interact with patients, coworkers and other healthcare professionals.
Ability to deal with a variety of patients in a wide range of situations
Ability to prioritize work in a high-volume work environment and work independently.
Ability to analyze problems, troubleshoot, and implement fixes for problems.
Ability to use computers, automated systems, and medical record software
Ability to utilize resources available to assist in pharmaceutical care
Knowledge of the laws relating to pharmacy practice
Knowledge of hospital or retail pharmacy procedures for filling, dispensing and billing prescriptions.
Ability to perform accurate work with careful attention to detail.
Work Schedule:
Per Diem, Every third weekend rotation Required Qualifications: Education: Currently enrolled in an accredited pharmacy program or a graduate of an accredited pharmacy program
Licensure: Must be registered as a pharmacy intern with Minnesota State Board of Pharmacy before starting. Preferred Qualifications: Pharmacy students finishing first or second professional year with ability to work a minimum of every other weekend rotating schedule plus one week night per week.
