Provide patient care based on nursing processes within your scope of practice during assigned shifts. Support patients' physical, psychosocial, educational, and safety needs appropriate to their age group. Gather and contribute accurate patient information to assessments, care plans, and discharge planning, involving patients and caregivers. Deliver compassionate care while maintaining organized nursing interventions and evaluations. Communicate professionally with patients, families, and healthcare team members to ensure quality care experiences. Adapt positively to rapid changes in departmental needs throughout the shift. Collaborate with physicians and team members regarding changes in patient conditions and diagnostic results. Perform therapeutic procedures as assigned and document nursing interventions and patient responses accurately. Provide individualized health education tailored to patient diagnoses. Assist with patient admissions, discharges, and transfers. Identify and address psychosocial needs, coordinating with social services and discharge planners. Recognize and manage potential health risks, initiating corrective actions in collaboration with RNs. Work with interdisciplinary teams to enhance patient care quality. Seek supervision or assistance when necessary to ensure safe and effective care. Maintain continuity of care including medication management and patient instruction. Utilize communication strategies to achieve positive outcomes and satisfaction. Leverage internal and external resources to resolve or prevent care-related issues. Education and Certifications:
Licensed Practical Nurse (LPN) credential required. Basic Life Support (BLS) certification required. Preferred certifications include Advanced Life Support, De-escalation techniques, Cardiopulmonary Resuscitation, and Pediatric Advanced Life Support.
