We are seeking an enthusiastic RN Care Manager to join a mission-driven care management team based in Everett, MA. This hybrid role offers the perfect blend of in-home visits and remote coaching-no night shifts, weekends, or on-call duties-giving you the work-life balance you deserve.
Key Responsibilities:
Conduct in-home visits to assess safety, monitor health conditions, and provide clinical coaching, including medication reviews. Follow structured care plans and offer ongoing support via a digital platform, including referrals and health coaching. Build supportive, trusting relationships with family caregivers to drive positive health outcomes and prevent burnout. Maintain timely and accurate documentation in compliance with regulatory standards. Participate in special projects and team initiatives as needed.
Qualifications:
Active Registered Nurse (RN) license in Massachusetts. Minimum of 2 years' recent experience in eldercare or supporting adults with disabilities. Valid driver's license and auto insurance; ability to travel for in-home visits. Strong background in case management, care planning, and assessments in complex care settings. Proficiency with MS Office and case management software. Bilingual candidates are encouraged to apply-language skills may positively impact compensation.
