Report to the Program Director and Nurse Coordinators. Working as part of a team in a residential program for fragile elderly residents to provide practical support and assistance with activities of daily living as well as emotional and social support to program residents. Assisting program case managers and program coordinator. Make regular contact with residents to ensure safety and well-being and to monitor needs for support. Become and remain fully familiarized with each resident's situation and needs through review of intake documents, daily log, case management notes, staff meetings and interactions with residents. Assisting residents with personal care (e.g. dressing, bathing, lifting, and transferring as well as with practical support needs (cooking, cleaning, laundry shopping). Accurately record in daily logbook all activities performed with each resident, including personal care and practical support provided. Perform other duties as requested by supervisor.

Requirements:
High school diploma or GED required; some college preferred. Certified Nurse's Aide or Home Health certification preferred. Experience in any of the following areas desirable: Special needs residential work, hospice, and home health care, experience working with the elderly, substance abuse or mental health treatment. Excellent oral, interpersonal and written communication skills. Demonstrated awareness of and sensitivity to issues related to the elderly, mental illness, racial/ethnic diversity, and addiction. Energetic and self-motivated reliability and flexibility. Demonstrated knowledge of and sensitivity to issues related to HIV, substance abuse, poverty & homelessness, and other populations required. Background and driving record checks will be performed.
