Are you ready to make a Difference?JOB SUMMARY:As a psychiatric nurse, you will work closely with our team of mental health professionals to provide comprehensive care to individuals with serious and persistent mental health conditions (SPMI). You will be responsible for assessing, planning, implementing, and evaluating nursing care plans to promote mental health and wellbeing. You will also provide education to individuals, families, and caregivers on medication management, symptom recognition, and coping skills. Duties and Responsibilities include:
Conduct mental health assessments, nursing assessments, and screenings to determine individuals' care needs. Develop and implement nursing care plans based on individuals' mental health conditions. Administer medication and monitor its effects. Monitor and manage individuals' physical health needs, such as vital signs, blood sugar levels, and weight. Provide education to individuals and their families on medication management, symptom recognition, and coping skills. Provide case management services related to identifying treatment needs and connecting individual to resources. Collaborate with health (PCP) and other mental health professionals to ensure coordinated care. Maintain accurate and up-to-date documentation of individuals' care plans, progress, and outcomes. Participate in quality improvement initiatives to enhance the effectiveness of the program. Participate in treatment team meetings and provide weekly summaries to progress and barriers for individuals and their goals. Maintain productivity standards as assigned. Complete state required trainings as assigned.

Qualifying employer for Public Student Loan Forgiveness RequirementsMINIMUMQUALIFICATIONS:
Active Licensed Practical Nurse (LPN) license in the state of Georgia
Minimum of two years of nursing experience in a mental health setting
Knowledge of psychiatric medications and their effects
Ability to conduct mental health assessments and screenings
Strong communication and interpersonal skills to work effectively with individuals, families, and other healthcare professionals
Ability to work independently and as a team member in a fast-paced environment
Must have valid, unrestricted Georgia Driver's License
Ability to use Electronic Medical Records (EMR) PREFERRED QUALIFICATIONS:
Certification in psychiatric and mental health nursing
Experience working in a community-based mental health program
Experience providing care to individuals with severe and persistent mental illnesses Bilingual abilities a plus
