Responsibilities:
Reviews, processes, and modifies, as necessary, all medication orders to maintain a current and accurate medication profile. Supervises, directs and reviews all activities involved with the compounding, storing, inventory, labeling, recording, or preparing medications by pharmacy technicians. Monitors drug therapies to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic levels, drug interactions, and therapeutic duplication. Monitors the use of drugs for compliance with prescribing criteria, guidelines, formulary, and protocols established by Norwegian American Hospital. Detects, monitors, documents and reports adverse drug reactions and medication errors. Serves as a resource of drug information for all practitioners and patients (i.e. drug identification, side effects, adverse events, interactions, medication counseling, etc.). Participates in pharmacy quality assessment/performance improvement (QA/PI) projects and/or initiatives. Assures proper control, documentation, and record keeping of controlled substances. Ensures all state and federal regulatory requirements are maintained
Requirements: Graduate of an ACPE accredited college of pharmacy, Pharm D preferred. Consideration may be given to graduates of foreign colleges of pharmacy who are appropriately licensed. Illinois Pharmacist licensure required.
