ESSENTIAL DUTIES & RESPONSIBILITIES
Assists service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered.
Monitors service recipient progress in treatment and participate in treatment plan reviews as assigned.
Updates treatment plans as appropriate. Submits treatment plans at all stages for medical review, if applicable. (clinical/direct service) Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conducts assessment and write assessment summaries, as assigned. (clinical/direct service) Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.
Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. (clinical/direct service) Assist service recipient in making discharge plans and provide follow up services as assigned (clinical/direct service). Monitors self-medication protocols, monitors the dosage, side effects, use and documents patients' reactions. Provides medication training to the client or client's family or guardian to administer the client's medication, to monitor proper levels of dosage and watch for side effects. Obtains laboratory specimens and help doctors with patient examination and continuity of care arrangements with other providers Monitors urinalysis, breathalyzer, and other testing as assigned.
Meets and maintains applicable certification and licensure requirements.
KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods. Knowledge of guidelines for confidentiality Knowledge of applicable regulatory guidelines and codes, agency policies and procedures, and Federal and state laws pertaining to position - including SUPR rules Knowledge of main effects, side effects and interaction effects of a broad range of psychotropic mediations. Knowledge of both psychiatric and physiological causative bases for illness. QUALIFICATIONS Education Level
HS Diploma or GED Years of Experience
Staff utilizing electronic health records and/or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency.
All employees in the EMT position must successfully complete required background checks and pre-employment screening. Certification/Licensure
Certification as Emergency Medical Technician
Must have and maintain driver's license and good driving record.
