**Job Description:**
- Are you a passionate Registered Nurse looking for a new adventure? - Join our dynamic team as a Case Manager and make a real impact in the lives of patients and their families! - This position requires: - **2 years of recent acute hospital Case Management experience** in Care Coordination and discharge planning. - **Travel experience is a must;** however, we welcome first-time travelers with strong backgrounds! - **Charting proficiency in Epic** is required.
**Highlights of the Role:**
- **Shift:** Day 5x8-Hour (08:00 - 16:30) - **Patient Ratios:** Days 1:20 - **Float Requirements:** Yes, to meet department needs. - **Weekend Coverage:** Required every other weekend. - **Daily Census:** Manage a case load of up to 25 patients.
**Essential Skills:**
- Qualified candidates will have experience with: - Admission Criteria - Appeals and Denials - Benefits Eligibility - Care Coordination - Discharge Planning - Needs Assessment/Order DME - Regulatory compliance (HIPAA, CMS, The Joint Commission) - Experience with HMOs, IPAs, etc. - Advance directive, POLST, and protective services process

- Work with a diverse age group: Adolescents, Adults, Pediatrics, and Geriatrics.
