THE OPPORTUNITY We are seeking a dedicated and compassionate Licensed Practical Nurse to join our Healthcare Services team as an LPN Case Coordinator. In this vital role, you will make a meaningful difference in the lives of individuals with developmental disabilities by managing their healthcare needs, coordinating medical services, and ensuring the highest standards of care and safety. As an LPN Case Coordinator, you will oversee a caseload of residential homes, working directly with individuals, families, healthcare providers, and support staff to deliver comprehensive, person-centered healthcare coordination. This position offers the opportunity to utilize your clinical skills while building meaningful relationships and advocating for those who depend on your expertise. WHAT YOU'LL DO
Manage healthcare coordination for an assigned caseload through regular site visits, medical monitoring, and comprehensive oversight
Collaborate with physicians, pharmacists, and medical professionals to coordinate care and implement treatment plans
Ensure medication administration safety through regular audits, staff training, and accurate documentation
Provide training and support to direct care staff on healthcare procedures, medication administration, and infection control
Maintain compliance with Ohio Department of Developmental Disabilities (DODD) regulations and licensure requirements
Coordinate appointments and transportation to medical, dental, therapy, and psychiatric appointments Provides on-call phone consultation services for healthcare concerns outside of office hours
Build relationships with families and respond to their concerns with compassion and professionalism WHAT WE'RE LOOKING FORRequired Qualifications:
Graduate of an accredited school of nursing
Current State of Ohio LPN license in good standing
Valid Ohio Driver's License with acceptable driving record
Ability to lift up to 50 pounds and perform physical tasks Preferred Qualifications:
One year of nursing experience in developmental disabilities or IDD settings Experience with medication administration programs and training Knowledge of DODD regulations and compliance requirements
