Requirements - Current Licensed Practical Nurse (LPN) license
Roles & Responsibilities - Assist in identifying patient needs and priorities for care based on various factors including age and lifestyle. - Gather, observe, record, and communicate patient responses to nursing interventions. - Collaborate with licensed practitioners to modify plans of care as needed. - Implement nursing interventions and document care accurately and timely. - Communicate relevant patient information with the multidisciplinary team to ensure continuity of care. - Participate in patient education and discharge planning.
